Now that the East side electronic sign has been installed and in use for about 3 months, we would like to solicit feedback. As this sign affects those on the East side only, we are asking that only East side residents complete this form.

Any name and addresses of  people taking the survey that do NOT live on the East side of Choctaw Lake will be deleted and not used as a part of the data collection process.

The survey will close September 27, midnight October 4, midnight.
